By submitting this order form and completing payment, you acknowledge and agree to the following:
Orders must be submitted and paid in full by July 10, 2026. Orders received after this date may not be accepted.
Plaques will feature the official headshot and published Fayetteville Observer profile article provided through the Fayetteville's 40 Under 40 program. No custom photos, article edits, or design modifications will be accepted.
All sales are final. Due to the custom nature of these recognition plaques, refunds, cancellations, or exchanges cannot be provided after an order has been placed.
Book Black Women, Inc. reserves the right to make minor design adjustments necessary for production, including resizing or formatting content to fit the selected plaque dimensions.
Honorees selecting "Pick Up at Ceremony" are responsible for collecting their plaque at the designated distribution time. Any plaques not picked up may be collected at a later date by appointment.
By placing an order, you authorize Book Black Women, Inc. to use your official Fayetteville's 40 Under 40 headshot, profile information, and published article solely for the purpose of producing your commemorative plaque.
